Thank you for applying for life insurance through Ethos Technologies Inc. (“Ethos”). These authorizations and disclosures are a required part of the application process. If there's something you're not sure about, please reach out to us.

Thank you for applying for life insurance through Ethos Technologies Inc. (“Ethos”). We would be happy to answer any questions you may have. Ethos is an independent broker, not an employee or subsidiary of any insurance carrier, and is not authorized to make or modify policy contracts or to waive any requirements or any information that a carrier may request.
Underwriting
Once we receive your application, we will begin an evaluation process called underwriting to determine whether you are eligible for insurance and, if so, the rate you should pay for that insurance. We may find that we are unable to give you the insurance you have applied for or that we are able to give it to you only on a modified basis or at a rate greater than our lowest rate.
Your application will be our primary source of information; therefore, it must be true, complete, and accurate. You must inform us of a change to any answer in any part of your application before accepting delivery of a policy; in fact, you agree to do so when you sign your application. We may seek information from other sources to help us evaluate the information you give us on your application.

Contestability
We strongly urge you to review the completed application closely for accuracy. A claim may be denied, the policy may be void or your coverage may be lost if the application is incomplete or if it contains false statements or material misrepresentations. Any policy that may be issued will indicate when and under what circumstances it may be contested. Please be aware that if the application contains material misrepresentations or conceals material facts, and you submitted it with the intent to defraud or to facilitate fraud against us and/or one of our insurance carrier(s), you may also be guilty of insurance fraud, which is a crime. You must inform us of a change to any answer in any part of your application before accepting delivery of a policy; in fact, you agree to do so when you sign your application.
Replacement of Existing Coverage
If you intend to replace existing coverage, tell us of your intention and answer “yes” to the replacement question in the application; state law may require us to give you information that will help you compare the policy you are applying for with the policy you intend to replace. If you are undecided about keeping existing coverage, indicating an intention to replace existing coverage may help you get the information you need to make a decision. If you do replace existing coverage, the new policy may contain new suicide and contestable periods. The following would be considered replacement: you stop paying premiums on an existing policy or surrender an existing policy before or shortly after applying through us or you borrow from an existing policy to pay premiums for the insurance for which you are applying. State law may define replacement to include other situations. Ask us if you are unsure.

Insurance Information Practices
We will rely primarily on information provided by you. We may supplement that information with information from other sources such as medical professionals who have treated you. In some cases, we may ask a consumer reporting agency to collect information and submit an investigative consumer report to us as explained in this Notice under Federal Fair Credit Reporting Notice. You may request to be interviewed in connection with the preparation of this report.
In certain limited situations, we are allowed by law to disclose necessary items of personal information to third parties without your specific authorization.
You have the right to be told about, and receive copies if you wish, of items of personal information about you that appear in our files, including information contained in investigative consumer reports. You also have the right to seek correction of information you believe to be inaccurate.
We will send you a more detailed explanation of our information practices if you send us a written request. You may send your request to the Director of Underwriting, Ethos Technologies Inc., at the address listed on our website.
Federal Fair Credit Reporting Notice
As part of our underwriting, we may ask that an investigative consumer report be prepared. An independent source known as a consumer reporting agency will prepare the report. The report will typically include information as to your character, general reputation, mode of living, and personal characteristics. The agency may conduct personal interviews with your family, friends, neighbors, business associates, financial sources, or others with whom you are acquainted in order to get this information. If you write to us within a reasonable time after you receive this Notice, we will tell you whether or not a report was requested. If a report was requested, we will tell you the name, address, and telephone number of the agency to whom the request was made. Upon request, the agency will furnish information as to the nature and scope of its investigation. If you would like to inspect and to receive a copy of the report, you may do so by contacting the agency directly.
Additional Disclaimer
In connection with this insurance, we may obtain or use a credit-based insurance score in conjunction with information you disclose to prequalify your potential application. An insurance score uses information from your credit report to help predict how often you are likely to file claims and how expensive those claims will be. This insurance score comes from a credit rating agency and amounts to a soft inquiry. Upon the completion of questions, we will let you know if we are unable to prequalify you before you submit an application.
MIB Pre-notice
Information regarding your insurability will be treated as confidential. Ethos, its carriers, or its reinsurers may, however, make a brief report thereon to the MIB, Inc., formerly known as Medical Information Bureau, a not-for-profit membership organization of insurance companies, which operates an information exchange on behalf of its members. If you apply to another MIB, Inc. member company for life or health insurance coverage, or a claim for benefits is submitted to such a company, MIB, Inc., upon request, will supply each company with the information about you in its file. Upon receipt of a request from you, MIB, Inc. will arrange disclosure of any information in your file. Please contact MIB, Inc. at 866-692-6901 (TTY 866-346-3642). If you question the accuracy of information in MIB, Inc.’s file, you may contact MIB, Inc. and seek a correction in accordance with the procedures set forth in the Federal Fair Credit Reporting Act.
The address of MIB, Inc.’s information office is 50 Braintree Hill Park, Suite 400, Braintree, MA 02184-8734. Ethos, its carriers, or its reinsurers, may also release information from its file to other insurance companies to whom you may apply for life or health insurance, or to whom a claim for benefits may be submitted. Information for consumers about MIB, Inc. may be obtained on its website at www.mib.com.
In an effort to provide you with an appropriate life insurance product, Ethos may request multiple reports from MIB Inc. as part of your application. Please be aware that such supplementary requests may be viewed by subsequent life insurance carriers, should you apply through another provider.

If I am Medicare-eligible, I am requesting to speak with a licensed agent who is certified to discuss Prescription Drug, Medicare Supplement, & Medicare Advantage insurance plans. I understand that this does not enroll me in a Medicare plan, affect/change my current enrollment, or obligate me to enroll in any plans. I consent to Ethos providing application information about me to a licensed third party agent in order to discuss such plan(s).
- This section gives Ethos and its affiliates permission to communicate with you about your plan, coverage, and related information.
I consent to receive phone calls, emails, and text messages from Ethos Technologies, its employees, affiliates, agents, insurance carriers, or other third parties acting on its behalf (collectively, “Ethos”) regarding my application, life insurance products, surveys, research, and policy services at the phone number(s) I provided, even if a wireless number. I understand (i) calls or texts may be generated using an automatic telephone dialing system or prerecorded voice message for telemarketing purposes, (ii) that I may opt out at any time by contacting Ethos, (iii) that applicable wireless and text message fees and data rates may apply to sending and receiving text messages, and (iv) that to make a purchase I do not need to consent to this policy and can instead call Ethos directly at (415) 275-9050. I also agree to the Ethos Privacy Policy and Terms of Use.
- This section acknowledges that you give verbal approval to participate in the application process, and that you understand the application, questions, and related answers you shared throughout.
You will receive a copy of your application package including important notices and authorizations. In connection with this application for insurance, it is understood and agreed that you have read or have been read the application and all statements and answers made or contained in this application and any supplements thereto, copies of which shall be attached to and made a part of any policy to be issued, are true and complete to the best of your knowledge and belief and made to induce Ethos to issue an insurance policy. The statements and answers in the application are the basis for any policy issued through Ethos (the “Company”), and no information about you will be considered to have been given to the Company or its carriers unless it is stated in the application.
No agent or other person has power to: accept risk; make or modify contracts; make, void, waive or change any conditions or provisions of the application, policy or receipt, as applicable; waive any of Company’s or its carriers’ rights or requirements; waive any information the Company or its carriers request; discharge any contract of insurance; or bind the Company or its carriers by making promises respecting benefits upon any policy to be issued.
You agree, that except as provided in a Temporary Insurance Application and Agreement, if any, insurance will not begin unless all persons proposed for insurance are living and insurable as set forth in the application at the time a policy is delivered to and accepted by you and the first modal premium is paid.
- This section gives Ethos permission to send and share with you digital documents related to your policy and account.
From time to time, Ethos Technologies Inc. (“we”, “us”, or “Company”) may send you certain notices, agreements, acknowledgments, disclosures, or other documents, including those which may be required by law (collectively, “Documents”). Described below are the terms and conditions for providing to you such Documents electronically through the Ethos website and/or the Policy Portal system. Please read the information below carefully and thoroughly, and if you can access this information electronically to your satisfaction and agree to this Electronic Record and Signature Disclosure and Consent (“ERSD”), please confirm your agreement by proceeding with your application, including by providing your email address and clicking the “Next” and/or by clicking the “Activate My Policy” button(s) to agree to this Electronic Record and Signature Consent.
Getting paper copies
At any time, you may request from us a paper copy of any record provided or made available electronically to you by us. You will have the ability to download and print documents we send to you through the Ethos Policy Portal during and immediately after the signing session. After such time, if you wish for us to send you paper copies of any such documents from our office to you, you will be charged a $0.00 per-page fee. You may request delivery of such paper copies from us by following the procedure described below.
Withdrawing your consent
If you decide to receive Documents from us electronically, you may at any time change your mind and tell us that thereafter you want to receive required Documents only in paper format. How you must inform us of your decision to receive future Documents in paper format and withdraw your consent to receive Documents electronically is described below.
Consequences of changing your mind
If you elect to receive required Documents only in paper format, it will slow the speed at which we can complete certain steps in transactions with you and delivering services to you because we will need first to send the required Documents to you in paper format, and then wait until we receive back from you your acknowledgment of your receipt of such paper Documents.
All Documents will be sent to you electronically
Unless you tell us otherwise in accordance with the procedures described herein, we will provide electronically to you through the Ethos Policy Portal all required Documents that are required to be provided or made available to you during the course of our relationship with you. To reduce the chance of you inadvertently not receiving any notice or disclosure, we prefer to provide all of the required Documents to you by the same method and to the same address that you have given us. Thus, you can receive all Documents electronically or in paper format through the regular mail delivery system. If you do not agree with this process, please let us know as described below. Please also see the paragraph immediately above that describes the consequences of your electing not to receive delivery of the Documents electronically from us.
How to contact Ethos Technologies Inc.
You may contact us to let us know of your changes as to how we may contact you electronically, to request paper copies of certain information from us, and to withdraw your prior consent to receive Documents electronically as follows:

To advise Ethos Technologies Inc. of your new email address
To let us know of a change in your email address where we should send Documents electronically to you, you must send an email message to us at support@getethos.com and in the body of such request you must state: your previous email address and your new email address. We do not require any other information from you to change your email address.
To request paper copies from Ethos Technologies Inc.
To request delivery from us of paper copies of the Documents previously provided by us to you electronically, you must send us an email to support@getethos.com and in the body of such request you must state your email address, full name, mailing address, and telephone number.
To withdraw your consent with Ethos Technologies Inc.
To inform us that you no longer wish to receive future Documents in electronic format you may:
Decline to sign a document from within your signing session, and on the subsequent page, select the check-box indicating you wish to withdraw your consent, or you may;
Send us an email to support@getethos.com and in the body of such request you must state your email, full name, mailing address, and telephone number. We do not need any other information from you to withdraw consent. The consequences of your withdrawing consent for online documents will be that transactions may take a longer time to process.
Required hardware and software
To access and retain documents, you must:
- Have access to a computer capable of running a current Internet browser;
- Be able to view all documents on a monitor through the use of Adobe Reader or similar software and be able to print all such documents; and
- Have access to Internet service and an email account that can be used to receive and access all documents;
- Be able to send and receive emails that contain hyperlinks to websites.
If you are accessing the Ethos Policy Portal electronically through a mobile device, such as a tablet, smartphone or similar device, you must be able to print and save the transmitted Documents. You can find apps that support printing and saving for most mobile devices through your mobile device’s app store. If your mobile device does not have this functionality, you must access the Ethos Policy Portal through alternate means that provide you with the ability to print and save the Documents. You acknowledge that you can access the electronic Documents in the designated formats described herein.
Acknowledging your access and consent to receive and sign Documents electronically
To confirm to us that you can access this information electronically, which will be similar to other electronic Documents that we will provide to you, please confirm that you have read this ERSD, and (i) that you are able to print on paper or electronically save this ERSD for your future reference and access; or (ii) that you are able to email this ERSD to an email address where you will be able to print on paper or save it for your future reference and access. Further, if you consent to receiving Documents exclusively in electronic format as described herein, then proceed with your application, including by providing your email address and clicking the “Next” and/or by clicking the “Activate My Policy” button(s).
By proceeding with your application, including by providing your email address and clicking the “Next” and/or by clicking the “Activate My Policy” button(s), you confirm that:
- You can access and read this ERSD;
- You can print on paper this ERSD, or save or send this ERSD to a location where you can print it, for future reference and access; and
- Until or unless you notify the Company as described above, you consent to receive exclusively through electronic means all Documents that are required to be provided or made available to you by the Company during the course of your relationship with us.
